Please FILL OUT Mentor Application below and click SUBMIT when you are finished.

We will contact you via email or phone once we receive your application. 

In addition, please PRINT OUT and COMPLETE BY HAND the following documentation:

1.  Sonoma Valley Voluntary Driver Form - /home/standbyme/C-Voluntary%20Driver-FORM%20C.pdf C-Voluntary Driver-FORM C.pdf

2.  Live Scan Fingerprint Form - /home/standbyme/FingerPrinting.pdf FingerPrinting.pdf  (please follow the enclosed instructions for completion)

Please bring the Driver Form with you to your scheduled Mentor Training.

Please complete the Fingerprint Form and bring it with you to your fingerprint appointment at the Sonoma Police Station.

***There is a $15 charge for fingerprinting which we will reimburse you for if you bring us the receipt**
